Aerial Integrative Health, PLLC is a cash-based practice. Full payment is due at time of service for Initial Consultations and Follow-up Visits. Aerial Integrative Health accepts cash, credit cards (VISA, MasterCard, American Express, Discover), and personal checks* from established patients.

To receive Package discounts, full package payment is due at time of first booking. No refund will be provided for package appointments not taken. At this time, payment plans are unable to be accepted. Aerial Integrative Health reserves the right to change fees as necessary.

*Personal checks can be accepted from established patients. Checks need to be made out to Aerial Integrative Health. There will be a returned check fee of $25. In this event, payment is due within 15 days. If payment is not received within 15 days, an additional 1.5% monthly penalty will be added to the balance. Two returned checks within one year will result in the loss of being able to pay via check.

I do not bill insurance. However, a superbill (a summary of your appointment with CPT codes and all corresponding modifiers) can be provided upon request for you to submit directly to your insurance company for possible reimbursement. It is your responsibility to determine if your insurance company covers our services. I strongly advise you to contact your health insurance company for details about coverage for services provided by naturopathic physicians before your visit if this is important to you. If information beyond a coded bill is requested, I charge my regular physician billing rate to process those requests.

Dispensary items may not be covered by insurance. However, you may be able to submit to a health savings account. Your treatment plan may serve as a prescription for this purpose if needed.

Cancellations or reschedules need to be made 48 hours before the scheduled appointment time. When you make an appointment, I reserve that time just for you and prepare ahead of time for our visit. As such, I require at least 48 hours’ notice if you are unable to make your appointment or if you request to change it. Any change, cancellation, or missed appointment without a minimum 48-hour notice will result in a charge of 25% of the scheduled payment. Exceptions may be made for medical emergencies, severe or inclement weather, or other situations deemed acceptable by the physician. You will receive appointment reminders via email and/or text.

Unfortunately, none of the services offered by Aerial Integrative Health, PLLC are reimbursable by Medicare, including secondary policies, or Medicaid. If you have Medicare or Medicaid, all services and expenses will be an out-of-pocket cost, including any laboratory testing.

Please note that this office does not take medical emergency calls! If you have a medical emergency, call 911 or go directly to the nearest emergency room. I do not address urgent care or primary care issues. I recommend you have a primary care physician. If you have a serious health problem that requires immediate attention, you should call your other doctor(s), call 911, or have someone take you to the nearest hospital emergency room. If you notice an adverse effect from one of the components of your health plan, you should discontinue it immediately and then contact me.

Patients are not required to purchase prescribed supplements here; however, it is difficult to verify sourcing and quality when purchasing online. The following online dispensaries offer practitioner-grade supplements. Recommended and prescribed products for patients can be found on these HIPAA-compliant platforms, thus ensuring privacy. These dispensaries are available to anyone for browsing and purchasing. All items purchased receive a 20% discount when ordered through the following links.
