Initial Consultation (up to 90 minutes) - $280

Specialized training to help regulate your mind, body and behavior. Sessions include approaches such as clinical biofeedback training, HRV, breathwork, mindfulness-based therapies, and meditation to help rewire and balance your nervous system.
Follow-up Visit (up to 30 minutes) - $115
The Initial Combo Highly Recommended! - $415
A combined Initial Consultation and 60-minute Follow-up Visit that offers a $50 discount. This ensures continuity and follow-up for both the you and Dr. Meldrum as well as allowing ample time to develop a highly personalized treatment plan.
Committed Check-in - $490
Commit to your health goals by committing to five 25-minute Follow-up Visits with Dr. Meldrum. After an Initial Consultation, you and Dr. Meldrum can discuss whether the Committed Check-in Package is right for you. This is a great way to keep yourself on track and to hold yourself accountable for your health changes. It also offers an opportunity to ask questions while practicing your health skills. Package offers an $85 discount.
Ready to Commit Package - $830
The Ready to Commit Package is for the patient committed to making lasting health changes. After an Initial Consultation, you and Dr. Meldrum can discuss whether the Ready to Commit Package is right for you. Included are five 60-minute Follow-up Visits for additional physical exam, follow-up and repeat of labs as needed, individualized in-office treatment each session, individualized health coaching, and follow-through on comprehensive treatment plan. Package offers a $95 discount.
Foundations Package - $1760
The Foundations Package is designed for the person ready to take complete charge of their health by establishing foundational health habits. After an Initial Visit, you and Dr. Meldrum can discuss whether the Foundations Package is right for you. Visits will be personalized for you. Package includes eleven monthly 60-minute follow up visits. Depending on treatment plan, these visits may include physical exam, laboratory studies and review, individualized treatment and coaching, follow through on treatment plan, one-on-one sessions with a health motivator or Dr. Meldrum, physical medicine, movement, and/or meditation, and one 60-minute grocery store tour based on nutrition goals. Package offers a $275 discount.
Other expected costs may be laboratory tests, diagnostics tests, and/or prescribed supplements.
Home visits may be available in the 98038 zip code after an Initial Consultation and within 10 miles from 98038. Home visit fee is $25 in addition to the Visit Fee.
Aerial Integrative Health, PLLC is a cash-based practice. When you schedule the Initial Consultation, a credit card is held on file to hold the appointment for you. Full payment is due at time of service for Initial Consultations and Follow-up Visits. The credit card on file will be billed automatically at the conclusion of your appointment unless you state during the appointment that you would like to use another form of payment. Aerial Integrative Health accepts cash, credit cards (VISA, MasterCard, American Express, Discover), and personal checks* from established patients. To receive Package discounts, full package payment is due at time of first booking. No refund will be provided for package appointments not taken. At this time, payment plans are unable to be accepted. Aerial Integrative Health reserves the right to change fees as necessary.
*Personal checks can be accepted from established patients. Checks need to be made out to Aerial Integrative Health. There will be a returned check fee of $25. In this event, payment is due within 15 days. If payment is not received within 15 days, an additional 1.5% monthly penalty will be added to the balance. Two returned checks within one year will result in the loss of being able to pay via check.
I do not bill insurance. However, a superbill (a summary of your appointment with CPT codes and all corresponding modifiers) can be provided and you can submit it directly to your insurance company for possible reimbursement. It is your responsibility to determine if your insurance company covers our services. I strongly advise you to contact your health insurance company for details about coverage for services provided by naturopathic physicians before your visit if this is important to you. If information beyond a coded bill is requested, I charge my regular physician billing rate to process those requests.
Dispensary items may not be covered by insurance. However, you may be able to submit to a health savings account. Your treatment plan may serve as a prescription for this purpose if needed.
Cancellations or reschedules need to be made 48 hours before the scheduled appointment time. When you make an appointment, we reserve that time just for you. As I prepare ahead of time for my visit with you, I require at least 48 hours’ notice if you are unable to make your appointment or if you request to change your appointment. A change, cancellation, or missed appointment without a minimum 48-hour notice will result in a charge of 25% of the scheduled payment. Exceptions may be made for medical emergencies, severe or inclement weather, or other situations deemed acceptable by the physician. You will receive appointment reminders via your choice of email or text.
MEDICARE AND MEDICAID
Unfortunately, none of the services I offer are reimbursable by Medicare, including secondary policies, or Medicaid. If you have Medicare or Medicaid, all services and expenses will be an out-of-pocket cost including any laboratory testing.
Please note that this office does not take medical emergency calls! If you have a medical emergency, call 911 or go directly to the nearest emergency room. At this time, I do not address urgent care or primary care issues. I recommend that you have a primary care physician. If you have a serious health problem that requires immediate attention, you should call your other doctor(s), call 911, or have someone take you to the nearest hospital emergency room. If you notice an adverse effect from one of the components of your health plan, you should discontinue it and then contact me.
